Athletic Scholarship Agreement
I accept this award and agree to the following requirements in order to receive the Lake Land
College Athletic Scholarship:
- Maintain full-time student status and successfully complete a minimum of twelve (12) credit hours each semester (excluding summer). Athletic Scholarship credits include all courses attempted (including developmental) and may be different than my overall college credits.
- Obtain a 1.75 grade point average the first semester or term that I receive the Athletic Scholarship and either a 2.00 grade point average or a 2.00 cumulative grade point average every semester or term thereafter. The Athletic Scholarship grade point average is based on all courses attempted (including developmental) and may be different than my overall college grade point average.
- If I receive a Scholarship for summer and I enroll in fewer than 6 credit hours, I must successfully complete all of the credit hours in which I enroll. If I receive a Scholarship for summer term and enroll in 6 or more credit hours, I must complete at least 6 credit hours. I must also meet the Athletic Scholarship grade point average requirements as described above.
- I understand that the academic requirements to maintain my Athletic Scholarship may be more stringent than the minimum eligibility standards required by the NJCAA. If I do not meet the Athletic Scholarship academic requirements, my Scholarship will be discontinued even though I may still be eligible to participate on my team.
- If my Scholarship is discontinued due to failure to meet the academic requirements, I may be eligible to have the Scholarship reinstated for future semesters. To request reinstatement, I must meet with my coach and submit a Request for Athletic Scholarship Reinstatement immediately after receiving notification that my Scholarship was discontinued. To be considered for reinstatement, I will be required to obtain my coach's recommendation , pay all costs to enroll full-time the next semester or term and meet the academic standards at the end of that term.
- Remain in good standing on my team. If I leave the team prior to the first official game of my sport, I will lose my Scholarship and be responsible for all costs associated with the current semester as well as subsequent semesters. If I leave the team after the first official game of my sport, and I have met all requirements for the Athletic Scholarship, my scholarship will remain in effect for that semester; however, it will be discontinued for future semesters.
- The Athletic Scholarship covers tuition, service fees (including textbook rental), activity fees and course fees for credit courses. I will be responsible for the costs for workbooks, lab manuals or other course materials that must be purchased as well as any late fees or other special assessments. Students are encouraged to complete the Free Application for Federal Student Aid (FAFSA) each year to help cover additional costs of attendance. The Illinois Student Assistance Commission Monetary Award Program and all other scholarships and awards earmarked for tuition only, including Lake Land College Foundation awards and chargebacks from out-of-district students, will be utilized before the Athletic Scholarship.
