Request a Transcript

What is a transcript?

A transcript is the official record of all academic work completed at the College.

How do I request a transcript?

I order to protect the privacy of student academic records, students are required to request copies of their official transcript in writing. Students may fax or mail their request to the Admissions and Records Office, complete a request form in the Admissions and Records Office or at the Effingham Kluthe Center, or request an official transcript through the Internet Registration and Information System (IRIS). Unofficial transcripts can also be printed through IRIS.

Requesting a transcript through IRIS

An Unofficial Transcript

  • Current students should log in to the Internet Registration and Information System (IRIS).
  • Go to the Academic Profile section in the Students Menu.
  • Click the Unofficial Transcript link.
  • Choose CR Credit Transcript and submit.
  • Print your transcript.

An Official Transcript

  • Current students should log in to the Internet Registration and Information System (IRIS).
  • Go to the Academic Profile section in the Students Menu.
  • Click the Transcript Request link.
  • Complete the request information and submit.

Requesting an official transcript by mail or fax

  • Print the Transcript Request form. This form is also available in the Admissions and Records Office or at the Effingham Kluthe Center.
  • Complete the form. All requests must contain the student’s name, student ID or social security number, student’s signature and where the transcript should be mailed.
  • Mail or fax the form to the Admissions and Records Office at the following address:
    Lake Land College
    Attn: Admissions and Records
    5001 Lake Land Blvd.
    Mattoon, IL 61938
    Fax: 217-234-5390

Requesting an official transcript to be picked up in person

  • Print the Transcript Request form. This form is also available in the Admissions and Records Office or at the Effingham Kluthe Center.
  • Present a photo identification at the time of pick-up.
  • Transcripts being picked up may temporarily be  unavailable during peak enrollment periods or at the end of the semester. Older records which may be archived will take longer to produce. Students will be informed at the time of the request if there will be a delay.

What is the cost to receive a transcript?

Lake Land College does not charge a fee to provide student transcripts.

What if my name has changed?

As official records, transcripts are issued only with the legal name under which a student’s records are currently held. To change the legal name on the student transcript and other student records, a copy of one of the following documents verifying the current legal name must be submitted by mail or fax at the time of the transcript request: court order, government issued ID/driver’s license or revised birth certificate.

Can release of my transcript be withheld by the College?

The College may withhold official transcripts of individuals who have unpaid financial obligations to the College.
